When you or someone you care for is recuperating from illness or injury, learning to live with a disability or requires lifestyle assistance, it is comforting to know that home care is only a phone call away. Home care provides essential support in the home to anyone – regardless of age – who requires health care services or assistance with activities of daily living.
Home care services are provided free of charge to residents served by Southern Health-Santé Sud and can be arranged on a short-term basis or on an extended basis. This option provides an alternative to the long-term care system until it is no longer possible for a person to remain safely in their own home.
In addition, supportive services may include individual and family supportive counseling,assistance navigating the housing continuum,
health education, geriatric assessment, health surveillance, nursing, rehabilitation and personal care assistance.

- Individuals/families are responsible for their own health. Our role is to support or augment services available from family, community or other services.
- Individuals progress toward and remain in an optimum state of wellness in the familiar environment of their home and local community.
- All individuals requiring Home Care should have access to Home Care.
- Home care is an integral partner in regional community development.
Our mission is to ensure the provision of effective and responsive home health care services to support independent living in the community. When this is not viable, we seek to ensure co-ordination of admission to facility care.
