Southern Health-Santé Sud

A Personal Care Home (PCH) provides 24-hour, facility-based care for people who are no longer able to manage or to safely live at home. There are 22 personal care homes located throughout Southern Health-Santé Sud (including affiliate/contract corporations). 

The goals of a PCH are to provide high quality care that:

• promotes the person’s independence
• supports cultural and spiritual needs
• maintains a friendly, homelike environment
• values the input, care and support of family members.

Eligibility for PCH is determined through an assessment and application by a home care case coordinator.
