Southern Health-Santé Sud is committed to improving the way primary health care is delivered to help you live a healthier life! One method of delivery of primary health care is through primary health centres where you can access a team of health care providers and services, including family physicians, nurse practitioners, midwives, nurses, dietitians, pharmacists, mental health professionals, therapists and other health care providers.

In a primary care health centre, you will receive coordinated care and connections to other services, ensuring continuity and ease of movement across the system when another health care provider may be required. For example, if you visit your family doctor for a check-up related to your diabetes, you may also be referred to home care services or a dietitian for information about other aspects of your care needs.

The range of services in a primary care health centre will vary from one community to another… there is no 'one size fits all' model. Services are determined according to the community’s needs and may include, for example:

  • nurse caring for childcare of common illness and the management of ongoing (chronic) health problems
  • healthy child development
  • treatment of minor injuries
  • referrals to other health care providers/services (such as specialist care)
  • mental health
  • health promotion, illness and injury prevention
  • prenatal and post-natal support
  • primary care provider visit
  • rehabilitation services
  • end of life care
