Occurrence Reporting Requirements

Applicable to: Hospitals/Transitional Care Sites/ Personal Care Homes

In accordance with the Provincial Policy, Critical Incident Reporting and Management (HCS200.2), any patient/resident who has a COVID-19 healthcare acquired infection that results in a serious consequence i.e.  such as admission to an ICU/extended hospital stay or death, are to be reported as a Critical Incident on an occurrence report.

The occurrence report is to be forwarded to the Regional Coordinators - Patient Safety as soon as possible and no later than 24 hours after confirmation of a COVID-19 healthcare-acquired infection by Infection, Prevention & Control.  If notification is on a weekend and not during regular business hours, please contact the Portage District General Hospital at 204-239-2211 and ask the Senior Leader On-Call to be paged in accordance with the regional Senior Leader On-Call policy (ORG.1010.PR.003). 

Leads (or delegates) review and share the reporting requirements with all applicable staff.
